Summary
This case report details a rare recurrent intramuscular ganglion cyst in the peroneus longus muscle. Complete surgical excision successfully treated the cyst, preventing recurrence and preserving nerve function.

Background:
- Ganglion cysts are common benign cystic lesions, typically found in the hand and wrist.
- Intramuscular ganglion cysts are rare, and those arising from the peroneus longus muscle are exceptionally uncommon.
Observation:
- A 45-year-old male presented with a two-year history of a recurrent intramuscular ganglion cyst within the peroneus longus muscle.
- Previous drainage surgery one year prior resulted in recurrence within one month.
- Magnetic resonance imaging confirmed a well-defined, lobulated fluid collection consistent with a ganglion cyst in the proximal peroneus longus.
Findings:
- Surgical exploration identified an encapsulated mass within the peroneus longus muscle belly.
- Complete excision of the ganglion cyst was performed and confirmed by histology.
- Postoperative follow-up at two and six months showed no recurrence and normal neurological function.
Implications:
- Ganglion cysts originating from the peroneus longus can cause leg swelling due to common peroneal nerve compression.
- Complete surgical excision, with meticulous nerve preservation, offers excellent outcomes for these rare intramuscular cysts.
