The evaluation stage signals the end of the nursing process. The nurse gathers evaluative data to assess whether or not the patient has attained the expected results. Background:

Community-dwelling older adults frequently utilize emergency departments (EDs).
Interprofessional Collaborative Practice (IPCP) models aim to improve care coordination and reduce healthcare costs.
The Geriatric Outreach and Training with Care! (GOT Care!) program was implemented to address these issues.

Purpose of the Study:

To evaluate the impact of the IPCP intervention (GOT Care!) on ED visit rates among community-dwelling older adults.
To explore stakeholder perspectives on the effectiveness and implementation of the IPCP intervention.
To identify demographic factors influencing the intervention's success.

Main Methods:

Convergent parallel mixed-methods design.
Quantitative analysis included paired-samples t-tests, Poisson regression, and generalized Poisson regression for ED visit data.
Qualitative analysis involved content analysis of open-ended survey responses from stakeholders.

Main Results:

A 26% reduction in ED visits was observed among the 51 older adult participants.
IPCP interventions demonstrated a potentially greater impact on older adults with polypharmacy, diabetes, and prior ED use.
