Summary

Objective quantitative evaluation of amyloid positron emission tomography (PET) using a PET-only method shows high concordance with visual assessment. This quantitative approach offers a useful, objective alternative for amyloid PET image interpretation.

Background:

  • Visual evaluation of amyloid positron emission tomography (PET) is standard but subjective.
  • Objective quantitative methods are needed for improved amyloid PET image interpretation.

Purpose of the Study:

  • To assess the utility of a PET-only quantitative method for amyloid PET.
  • To compare quantitative amyloid PET evaluation with standard visual assessment.

Main Methods:

  • Retrospective analysis of 166 participants (controls, mild cognitive impairment, early Alzheimer's disease) from J-ADNI.
  • 11C-Pittsburgh compound-B (PiB) PET imaging was used.
  • Amyloid accumulation was assessed via visual review and quantitative analysis using standardized uptake value ratio (SUVR) with an adaptive template and PiB-prone regions.

Main Results:

  • Visual evaluation and SUVR showed significant correlation (ρ = 0.85-0.87, p < 0.05).
  • Visual evaluation achieved 78% sensitivity, 76% specificity, and 77% accuracy.
  • Quantitative evaluation yielded 77-79% sensitivity, 79% specificity, and 78-79% accuracy.

Conclusions:

  • The PET-only quantification method demonstrated concordance with visual evaluation.
  • This quantitative approach is considered useful for objective amyloid PET interpretation.
