Background:

Rectal cancer surgery can lead to erectile dysfunction (ED).
Low anterior resection (LAR) and Miles' procedures are common surgical options.
Comparing ED rates after these procedures is crucial for patient outcomes.

Purpose of the Study:

To compare postoperative erectile dysfunction rates between Miles' procedure and LAR.
To assess the efficacy of tadalafil in treating ED post-rectal cancer surgery.

Main Methods:

Prospective assessment of 50 rectal cancer patients (Stages 1-2).
25 patients underwent Miles' procedure, 25 underwent LAR.
International Index of Erectile Function (IIEF) assessed preoperatively and 6 months postoperatively.

Main Results:

No significant preoperative IIEF score differences.
Postoperative IIEF scores significantly decreased in both groups.
Miles' procedure resulted in significantly lower mean postoperative IIEF scores compared to LAR.
Tadalafil improved IIEF scores in patients with ED (IIEF ≤ 25) at 6 months.

Conclusions:

Miles' procedure is associated with a higher rate of erectile dysfunction post-rectal cancer surgery compared to LAR.
Postoperative erectile dysfunction significantly impacts patients' quality of life.
Holistic patient care should address both oncological and functional outcomes after rectal cancer surgery.
