  • A 24-month randomized trial with an embedded realist evaluation was conducted in 24 nursing homes across four European countries.
  • Data from 183 observations (335 hours) were triangulated with interview transcripts and context survey data from 357 staff interviews and 725 questionnaires.

  • Observations provided unique contextual details not fully captured by surveys or interviews.
  • Observations highlighted the impact of resources (staffing, supplies) and leadership (formal/informal, teamwork, autonomy) on knowledge use.
  • Observations illuminated the role of standards and the physical environment, as well as dynamic contextual interactions, in evidence-based practice.

Conclusions:

  • Despite being resource-intensive, qualitative observations significantly enhance understanding when combined with other data collection methods.
  • Observations deepen insights into the implementation context and its influence on evidence use and sharing.
  • Observations are key to improving the understanding of the context-evidence use-knowledge sharing triad in implementation research.
