Electrocardiogram01:29

Electrocardiogram

5.0K
An electrocardiogram (ECG or EKG) is a critical diagnostic tool that records the electrical signals produced by the heart during each heartbeat. This recording is achieved through electrodes placed strategically on the arms, legs, and chest. The electrocardiograph amplifies these signals and produces 12 distinct tracings, offering a comprehensive understanding of the heart's electrical activity.
Three major waveforms are present in a typical ECG recording: the P wave, the QRS complex, and...

Exercise Stress Test01:26

Exercise Stress Test

848
Introduction
Exercise stress testing, commonly known as a treadmill test, is a noninvasive procedure used to evaluate cardiovascular function and diagnose heart conditions.
Definition
An exercise stress test measures the heart's response to exertion using a treadmill or stationary bicycle. Chest electrodes record the heart's electrical activity through an ECG, and blood pressure is monitored regularly.

Anorexia Nervosa01:28

Anorexia Nervosa

681
Anorexia nervosa is a complex and severe eating disorder characterized by an intense fear of weight gain, an unrelenting pursuit of thinness, and a distorted body image. It often leads to dangerously low body weight relative to an individual's age and height. This disorder is marked by significant physical and psychological consequences, making it one of the most life-threatening psychiatric illnesses.
Symptoms and Physical Effects
Individuals with anorexia nervosa commonly exhibit extreme...

Disturbances in Heart Rhythm01:29

Disturbances in Heart Rhythm

2.2K
Arrhythmia or dysrhythmia refers to an abnormal heart rhythm caused by a defect in the heart's conduction system. It can cause the heart to beat irregularly, too quickly, or too slowly, leading to symptoms like chest pain, shortness of breath, and fainting. Factors such as stress, caffeine, alcohol, nicotine, cocaine, certain drugs, congenital defects, diseases, and electrolyte abnormalities can trigger arrhythmias.
Arrhythmias are categorized by their speed, rhythm, and origin. A slow heart...

Cardiac Action Potential01:30

Cardiac Action Potential

5.1K
Cardiac action potentials are essential for proper heart function, enabling the rhythmic contractions needed for adequate blood circulation. Nodal cells and Purkinje fibers, specialized for electrical conduction, generate these action potentials.
The cardiac action potential process involves a series of phases characterized by the movement of ions across the cardiac cell membranes, leading to the depolarization and repolarization of the cardiac myocytes.
Ionic Basis of Cardiac Action Potentials

ECG Interpretation of Arrhythmias II: Atrial, Junctional and Ventricular Arrhythmias

337
Arrhythmia is a condition characterized by an irregular heart rhythm, with ECG changes that differ based on its origin and nature. The types of arrhythmias discussed below include atrial, junctional, and ventricular arrhythmias.Atrial ArrhythmiasPremature Atrial Complexes (PACs): PACs are early atrial beats caused by stress, caffeine, alcohol, electrolyte imbalances, hypoxia, hyperthyroidism, or certain medications (e.g., bronchodilators and decongestants). The ECG shows early P waves with an...

Summary

Anorexia Nervosa patients show abnormal heart rhythm responses during exercise, with their QT interval failing to shorten unlike healthy individuals. This prolonged QT interval during exertion may explain the heightened risk of sudden cardiac death in Anorexia Nervosa.

Area of Science:

  • Cardiology
  • Psychiatry
  • Internal Medicine

Background:

  • Anorexia Nervosa (AN) is a severe eating disorder associated with significant mortality.
  • Electrocardiogram (ECG) abnormalities, particularly QT interval changes, are implicated in sudden cardiac death in AN patients.
  • The precise nature of QT interval dynamics during physiological stress, such as exercise, remains underexplored in AN.

Purpose of the Study:

  • To investigate and characterize QT interval behavior during a structured exercise test in individuals with Anorexia Nervosa.
  • To compare exercise-induced QT interval changes in AN patients with those of age- and sex-matched healthy controls.

Main Methods:

  • A prospective cohort study involving 18 Anorexia Nervosa patients and healthy controls.
  • A 6-minute modified exercise test protocol was administered.
  • A single-lead ECG patch recorded heart rate (HR) and QT interval, with QTc calculated for analysis.

Main Results:

  • Heart rate responses to exercise were comparable between AN patients and controls.
  • While healthy controls exhibited a shortening of the corrected QT (QTc) interval at maximum heart rate, AN patients demonstrated significant QTc prolongation.
  • Baseline and end-recovery QTc intervals were similar between the groups, but the dynamic change during exercise differed markedly.

Conclusions:

  • Anorexia Nervosa patients exhibit an impaired ability to shorten their QTc interval during exercise, unlike healthy individuals.
  • This blunted QTc response during exertion may be a key mechanism contributing to the increased risk of fatal arrhythmias and sudden cardiac death observed in AN.
  • Further research into cardiac electrophysiology in AN is warranted to understand and mitigate these risks.
