Summary

Platelet distribution width (PDW) may predict lupus nephritis (LN) in patients with systemic lupus erythematosus (SLE). Low PDW combined with high 24-hour urine protein shows diagnostic value for LN.

Background:

  • Lupus nephritis (LN) is a significant complication of systemic lupus erythematosus (SLE).
  • Identifying reliable biomarkers for LN prediction is crucial for early intervention.
  • Platelet distribution width (PDW) is an emerging hematological parameter.

Purpose of the Study:

  • To investigate the predictive value of platelet distribution width (PDW) in lupus nephritis (LN).
  • To assess the correlation of PDW with disease activity and clinical parameters in SLE patients.

Main Methods:

  • A retrospective study included 212 SLE patients and 201 healthy controls.
  • Clinical characteristics and laboratory data were collected from medical records.
  • Statistical analysis was performed to evaluate PDW's predictive capability.

Main Results:

  • PDW was significantly decreased in SLE patients and negatively correlated with SLEDAI-2K, disease duration, and 24-hour urine protein.
  • Low PDW group showed higher SLEDAI-2K, 24-hour urine protein, and anti-dsDNA antibody levels.
  • PDW demonstrated an area under the ROC curve of 0.87 for LN prediction, with 87.41% sensitivity and 74.63% specificity.

Conclusions:

  • PDW is a potential predictive marker for lupus nephritis (LN).
  • Low PDW combined with high 24-hour urine protein is a valuable indicator for diagnosing LN.
  • PDW can aid in detecting LN even in cases with negative 24-hour urine protein.
