Male Sexual Response: Erection & Ejaculation

Sexual stimulation can take various forms, such as physical touch and visual or auditory cues. When this happens, the parasympathetic reflex in the sacral portion of the spinal cord is activated. This reflex stimulates the release of nitric oxide (NO), which then dilates the arterioles in the penis, increasing blood flow to the erectile tissues - the corpora cavernosa and corpus spongiosum.
The blood filling the erectile tissues compresses the veins, which helps to prevent blood from leaving...

Area of Science:

  • Sexual Medicine
  • Digital Health
  • Psychotherapy

Background:

  • Mobile health (mHealth) apps offer on-demand patient support, enhancing motivation and adherence in healthcare interventions.
  • The application of mHealth in sexual medicine remains underexplored, particularly for conditions like premature ejaculation (PE).

Purpose of the Study:

  • To assess the efficacy of integrating a mobile coaching app with psychological treatment for premature ejaculation (PE).
  • The study aimed to evaluate the impact of app-guided therapeutic exercises on PE management.

Main Methods:

  • A pilot study involving 35 heterosexual men diagnosed with primary psychogenic PE.
  • Participants received 15 sessions of psychodynamic psychotherapy combined with behavioral therapy.
  • Patients were randomized into two groups: treatment as usual (TAU) with verbal/printed instructions, and an app group receiving guided exercises via smartphone.

Main Results:

  • The mobile app group demonstrated statistically significant improvements in both the Premature Ejaculation Diagnostic Tool and Premature Ejaculation Profile scores compared to the TAU group (P < .01).
  • A significantly higher proportion of patients in the app group achieved a 'no-PE' condition post-treatment compared to the TAU group (P < .001).

Conclusions:

  • Integrating a mobile coaching app into psychological treatment significantly enhances outcomes for premature ejaculation (PE).
  • The app-guided intervention proved superior to standard care in improving ejaculatory delay and sexual self-confidence.
  • Further research should explore long-term effects and the integration of mobile coaching with pharmacotherapy.
