Patients with end-stage renal disease (ESRD) undergoing peritoneal dialysis (PD) showed decreased structural brain connectivity. Hemodialysis (HD) patients exhibited altered functional connectivity, suggesting different impacts of renal replacement therapies on brain networks.
Area of Science:
- Neuroscience
- Nephrology
- Medical Imaging
Background:
- End-stage renal disease (ESRD) affects multiple organ systems, including the brain.
- Dialysis, a treatment for ESRD, may impact neurological function.
- Understanding brain connectivity changes in different dialysis modalities is crucial.
Purpose of the Study:
- To compare structural and functional brain connectivity in ESRD patients on peritoneal dialysis (PD) versus hemodialysis (HD).
- To identify differences in brain network alterations based on the type of renal replacement therapy.
Main Methods:
- Diffusion tensor imaging (DTI) and resting-state functional magnetic resonance imaging (rs-fMRI) were used.
- Graph theoretical analysis was applied to structural and functional connectivity matrices.
- 40 ESRD patients (20 PD, 20 HD) and healthy controls were studied.
Main Results:
- PD patients showed significantly decreased global structural connectivity (efficiency) compared to controls.
- HD patients exhibited increased characteristic path length and decreased small-worldness in functional connectivity.
- No significant structural connectivity differences were found in HD patients compared to controls.
- No significant functional connectivity differences were found in PD patients compared to controls.
Conclusions:
- Brain network alterations differ between PD and HD treatments in ESRD patients.
- The findings suggest that the type of renal replacement therapy influences brain network integrity.

Dialysis
Acute kidney injury develops suddenly and can be caused by pre-renal causes (e.g., hypovolemia, shock), intrinsic renal causes (e.g., acute tubular necrosis), or post-renal causes (e.g., urinary obstruction). In contrast, chronic renal failure progresses gradually over time and is often...
