Fatal generalized BCG histiocytosis.

    Summary

A child vaccinated with BCG developed a persistent Salmonella typhimurium infection, leading to disseminated abscesses. Post-mortem cultures confirmed Mycobacterium bovis BCG in widespread nodules, suggesting a potential link between BCG vaccination and severe Salmonella infections.

    Area of Science:

    • Immunology
    • Infectious Diseases
    • Microbiology

    Background:

    • Bacille Calmette-Guérin (BCG) vaccination is a standard procedure for infants to prevent tuberculosis.
    • Disseminated infections following BCG vaccination are rare but serious complications.

    Observation:

    • A pediatric case presented with recurrent Salmonella typhimurium infections and multiple abscesses starting at 3 years 10 months of age.
    • The patient experienced persistent fever, hepatosplenomegaly, and abscesses in various locations, including peri-nephritic, lymph nodes, paravertebral, and retro-peritoneal.
    • Post-mortem examination revealed widespread histiocytic nodules containing Mycobacterium bovis BCG.

    Findings:

    • Salmonella were consistently isolated from abscesses throughout the infection.
    • Mycobacterium bovis BCG was cultured from disseminated histiocytic nodules identified during post-mortem examination.
    • A familial link was noted, with a previously reported similar case involving a relative.

    Implications:

    • This case highlights a potential, albeit rare, association between BCG vaccination and severe, disseminated Salmonella infections.
    • Further investigation into host immune responses, particularly in familial clusters, may be warranted to understand susceptibility.
    • Understanding such rare adverse events is crucial for vaccine safety monitoring and public health.
