Observation:

  • A 76-year-old, postmenopausal, sexually inactive female presented with a 10-month history of voiding difficulty, postmicturition dribbling, and urinary leakage.
  • Examination revealed near-total labial fusion causing urethral and vaginal occlusion with only a 3-mm posterior opening.

Findings:

  • Surgical separation of labial adhesion, combined with estrogen cream and self-dilatation, successfully treated the patient.
  • The patient experienced no surgical complications and showed no recurrence of labial adhesion or voiding dysfunction at 6-month follow-up.

Implications:

  • This case highlights the importance of thorough genital examinations in diagnosing the cause of urological symptoms in elderly females.
  • Prompt diagnosis and intervention for labial adhesion can effectively resolve voiding dysfunction and improve quality of life.
