Background:

Voiding difficulty is common in males but can affect females due to various anatomical and neurogenic factors.
Labial adhesion is a rare but significant cause of female voiding dysfunction, particularly in the elderly.
The incidence of labial adhesion in the elderly population remains under-elucidated.

Observation:

A 76-year-old, postmenopausal, sexually inactive female presented with a 10-month history of voiding difficulty, postmicturition dribbling, and urinary leakage.
Examination revealed near-total labial fusion causing urethral and vaginal occlusion with only a 3-mm posterior opening.

Findings:

Surgical separation of labial adhesion, combined with estrogen cream and self-dilatation, successfully treated the patient.
The patient experienced no surgical complications and showed no recurrence of labial adhesion or voiding dysfunction at 6-month follow-up.

Implications:

This case highlights the importance of thorough genital examinations in diagnosing the cause of urological symptoms in elderly females.
Prompt diagnosis and intervention for labial adhesion can effectively resolve voiding dysfunction and improve quality of life.
