The sphenoid sinus is less frequently involved in chronic rhinosinusitis without nasal polyps (CRSsNP) compared to chronic rhinosinusitis with nasal polyps (CRSwNP). This study highlights a key difference between these CRS subtypes.

Background:

  • Chronic rhinosinusitis (CRS) is a prevalent chronic condition with two primary subtypes: CRS with nasal polyposis (CRSwNP) and CRS without nasal polyps (CRSsNP).
  • The sphenoid sinus is often considered less involved in CRSsNP compared to CRSwNP.

Purpose of the Study:

  • To compare the incidence of sphenoid sinus involvement between CRSsNP and CRSwNP.
  • To investigate potential correlations between sphenoid sinus involvement and clinical variables in CRS patients.

Main Methods:

  • Retrospective chart review of 289 CRS cases, with 151 meeting inclusion criteria.
  • Assessment of clinical and imaging findings including age, sex, adenoid and inferior turbinate hypertrophy, nasal septum deviation, polyps, and Lund-McKay scores.
  • Statistical analysis to determine the incidence of sphenoid sinus involvement in CRSwNP and CRSsNP and its correlation with other variables.

Main Results:

  • Sphenoid sinus involvement was significantly higher in CRSwNP (89%) compared to CRSsNP (65.2%) (P=.0001).
  • No statistically significant differences in the involvement of other paranasal sinuses were observed between the two phenotypes.
  • Age, gender, nasal septum deviation, inferior turbinate hypertrophy, and adenoid hypertrophy did not significantly correlate with sphenoid sinus involvement.

Conclusions:

  • This study is the first to demonstrate a lower incidence of sphenoid sinus involvement in CRSsNP.
  • Further research is warranted to elucidate the factors contributing to the reduced sphenoid sinus involvement in CRSsNP.
