Deep brain stimulation (DBS) of the habenula (HB) showed initial symptom improvement in two patients with treatment-resistant schizophrenia. However, only one patient maintained benefits long-term, indicating variable efficacy for this novel therapy.
Area of Science:
- Neurosurgery
- Psychiatry
- Neurology
Background:
- Schizophrenia presents significant morbidity and mortality.
- Current treatments like antipsychotics and ECT are ineffective for 10-30% of patients.
- Deep brain stimulation (DBS) is a promising intervention for psychiatric disorders.
Purpose of the Study:
- To explore the utility of habenula (HB) DBS in managing severe, chronic, treatment-resistant schizophrenia.
- To assess the safety and efficacy of HB DBS in a clinical setting.
Main Methods:
- Two young adult males with treatment-resistant schizophrenia underwent HB DBS surgery.
- Clinical symptoms were monitored post-surgery for improvements and adverse effects.
Main Results:
- Both patients showed initial symptom improvement within 6 months post-HB DBS.
- One patient maintained clinical benefits at 12-month follow-up.
- The other patient experienced symptom worsening, requiring hospitalization and study withdrawal.
Conclusions:
- HB DBS may be a safe and effective surgical option for select patients with treatment-resistant schizophrenia.
- Long-term outcomes and patient selection criteria for HB DBS require further investigation.
- Individual patient responses to HB DBS can vary significantly.
