  • Chest computed tomography (CT) is a crucial imaging modality for evaluating suspected COVID-19 pneumonia.

Purpose of the Study:

  • To evaluate the interobserver variability among chest radiologists in interpreting chest CT findings for COVID-19 pneumonia.
  • To assess the reliability of the Radiological Society of North America (RSNA) expert consensus statement reporting guidelines.

Main Methods:

  • Three fellowship-trained chest radiologists reviewed 303 chest CT studies of patients with suspected COVID-19.
  • Radiologists classified findings as typical, indeterminate, atypical, or negative for COVID-19 pneumonia based on RSNA guidelines.
  • Interobserver agreement was analyzed using Fleiss kappa and Cramer V statistics.

Main Results:

  • Almost perfect interobserver agreement was observed for typical (kappa=0.815), atypical (kappa=0.806), and negative (kappa=0.962) COVID-19 appearances.

Conclusions:

  • The RSNA expert consensus statement provides a reliable framework for reporting chest CT findings in suspected COVID-19 pneumonia.
  • The guidelines demonstrate substantial to almost perfect interobserver agreement among radiologists.
  • This framework facilitates accurate communication of suspicion levels based on objective findings.
