Background:

The transition from pediatric to adult epilepsy care is gaining attention for its potential to improve patient outcomes.
Existing literature primarily focuses on program structure, lacking clinical data during the transition period.
This study addresses this gap by presenting pilot data from a multidisciplinary transition program.

Observation:

Pilot data were collected from 28 participants attending the initial visit of a transition program at a level 4 epilepsy center.
Nearly all participants (96%) had a history of neurobehavioral comorbidities, with anxiety, ADHD, and depression being most common.
A significant majority (77%) reported current neurobehavioral issues impacting psychosocial functioning.

Findings:

The most frequent recommendations included enhancing independence in epilepsy management (64%) and self-care/independent living (82%).
Psychological intervention (43%) and increased socialization (43%) were also key recommendations.
A case example illustrated the program's process and initial outcomes.

Implications:

Multidisciplinary care, including psychosocial providers, is vital for a successful transition from pediatric to adult epilepsy services.
Addressing neurobehavioral comorbidities is essential for optimizing psychosocial functioning during healthcare transitions.
These findings underscore the importance of tailored support systems for adolescents and young adults with epilepsy.
