    Summary

    Background:

    • Glaucoma and ocular hypertension are conditions characterized by elevated intraocular pressure (IOP).
    • Effective IOP management is crucial to prevent optic nerve damage and vision loss.

    Purpose of the Study:

    • To evaluate the efficacy of 4% atenolol eye drops (Tenormin) in reducing IOP.
    • To assess the duration of action and sustained effectiveness of atenolol eye drops.

    Main Methods:

    • A double-masked, cross-over trial involving patients with open-angle glaucoma, ocular hypertension, and closed-angle glaucoma.
    • Intraocular pressure was measured using tonometry after administration of 4% atenolol eye drops or saline control.
    • Statistical analysis was performed using the Wilcoxon matched pairs rank test.

    Main Results:

    • A median fall of 5-6 mmHg in IOP was observed in the first tonometried eyes, which was statistically significant (P < 0.05).
    • A median IOP reduction of 3-5 mmHg was noted in the second tonometried eyes, also statistically significant (P < 0.05).
    • In patients treated for 1-2 months, replacing atenolol with saline resulted in an IOP increase of approximately 3 mmHg several days later, indicating sustained drug effect.

    Conclusions:

    • 4% atenolol eye drops are effective in lowering intraocular pressure in patients with glaucoma and ocular hypertension.
    • The cardioselective beta-blocker atenolol demonstrates sustained IOP-lowering effects for at least one to two months.
