This study presents a rare, benign atypical sellar cyst with unique histological features not previously documented in neurosurgery literature. Further research is needed to classify these unusual sellar lesions.

Background:

  • Sellar cysts are common, with 90% being pituitary adenomas.
  • Nonadenomatous sellar cysts include inflammatory, infective, metastatic, and rare types like dermoid, epidermoid, colloid, and arachnoid cysts.

Observation:

  • A 60-year-old female presented with vision blurring and headache.
  • Imaging revealed a bony erosive sellar lesion with high MRI signals and no contrast enhancement.
  • Radiology could not provide a definitive diagnosis.

Findings:

  • Histopathological analysis diagnosed a benign atypical sellar cyst.
  • The cyst exhibited unique features not previously documented in existing literature.
  • The patient recovered well after surgical resection.
