Summary

In super-elderly patients, electroencephalogram (EEG) δ-θ bicoherence increases with sevoflurane anesthesia, unlike α activity. This finding suggests δ-θ bicoherence may aid anesthesia monitoring in this population.

Background:

  • Electroencephalograms (EEGs) change with age, but their response to anesthesia in the very old is not well understood.
  • Previous studies have not detailed anesthesia-dependent oscillatory features or EEG reactivity in super-elderly patients.

Purpose of the Study:

  • To investigate age-related differences in EEG oscillatory features during sevoflurane anesthesia.
  • To examine anesthesia-dependent changes in EEG bicoherence and power spectrum in super-elderly versus young adult patients.

Main Methods:

  • Compared frontal EEG oscillatory features (bicoherence, power spectrum in α and δ-θ bands) in 20 super-elderly (87.1 years) and 20 young adult patients.
  • Analyzed EEG at three sevoflurane anesthesia levels (0.3, 0.7, 1.4 MAC) using the Friedman test.

Main Results:

  • Super-elderly patients showed anesthesia-dependent increases in δ-θ bicoherence (p=0.000), while α bicoherence remained unchanged (p=0.142).
  • Young adults exhibited anesthesia-dependent changes in both δ-θ and α bicoherence, contrasting with the super-elderly group.

Conclusions:

  • In the super-elderly, δ-θ bicoherence of EEG demonstrates anesthesia-dependent changes, whereas α activity is minimal regardless of anesthesia level.
  • Quantification of δ-θ bicoherence is proposed as a potential method for anesthesia monitoring in super-elderly individuals.
