Background:

  • Two middle-aged men with insulin-dependent diabetes presented with progressive lower limb pain and numbness.
  • Clinical examination revealed mixed upper and lower motor neuron signs and sensory loss below T9-10.
  • Cerebrospinal fluid (CSF) showed mild protein elevation and pleocytosis in one case.

Findings:

  • The cases presented with focal, rather than diffuse, myelopathy.
  • Nerve conduction studies indicated a concurrent mild sensorimotor neuropathy.
  • Extensive investigations ruled out other potential causes of myelopathy.

Implications:

  • These findings challenge the notion that diabetic myelopathy is exclusively diffuse.
  • Focal myelopathy should be considered in the differential diagnosis of spinal cord disorders in diabetic patients.
  • Further research is needed to elucidate the pathogenesis and diagnostic criteria for diabetic myelopathy.
