Summary

The Perceval valve causes a significant drop in platelet count after aortic valve replacement compared to the Intuity valve. This decrease in platelets did not increase bleeding risk, suggesting cautious use of platelet transfusions.

Background:

  • Postoperative thrombocytopenia is common after aortic valve replacement (AVR) with extracorporeal circulation.
  • The Perceval valve has been linked to a more pronounced drop in platelet count than other bioprostheses.
  • This study compares platelet count changes between Perceval and Intuity valves during AVR.

Purpose of the Study:

  • To analyze and compare platelet count progression after isolated AVR using Perceval versus Intuity valves.
  • To evaluate the impact of platelet transfusion on platelet count recovery in patients receiving these valves.

Main Methods:

  • Retrospective analysis of 80 patients receiving Perceval valves and 141 patients receiving Intuity valves for isolated AVR (March 2014 - December 2018).
  • Patients were categorized based on whether they received platelet transfusions.
  • Platelet count nadir and recovery rates were compared between valve groups.

Main Results:

  • Patients with Perceval valves experienced a significantly greater drop in minimum platelet count (54% of preoperative) compared to Intuity valves (67%, p<0.001).
  • In transfused patients, minimum counts were 52% for Perceval and 79% for Intuity (p<0.01), with faster recovery for Intuity.
  • No significant differences in abnormal bleeding or packed red blood cell transfusion rates were observed between groups.

Conclusions:

  • The Perceval valve is associated with a significant, transient decrease in platelet count post-AVR compared to the Intuity valve.
  • This platelet drop does not appear to elevate bleeding risk.
  • Platelet transfusions in this context should be guided by clinical judgment rather than solely by absolute platelet values.
