    Background:

    • Sinus floor elevation (SFE) with the lateral window technique (LWT) is a common procedure to augment bone height in the posterior maxilla.
    • Sinus membrane perforation (SMP) is a potential complication during SFE, which may affect treatment outcomes.

    Purpose of the Study:

    • To identify and analyze patient-related and surgical-anatomical risk factors associated with sinus membrane perforation (SMP) during sinus floor elevation (SFE) using the lateral window technique (LWT).

    Main Methods:

    • A retrospective analysis of 434 SFE procedures using LWT was conducted.
    • Patient-related factors (age, sex, smoking, diabetes) and surgical-anatomical factors (staged approach, sinus side, residual ridge height, membrane thickness, previous surgeries, septa, biotype) were compared between perforated and non-perforated sites.
    • Risk factors for both small/moderate (<10 mm) and large (≥10 mm) SMPs were evaluated.

    Main Results:

    • SMP occurred in 23.8% of procedures.
    • No significant difference in patient-related risk factors was found between perforated and non-perforated sites.
    • Significant surgical-anatomical risk factors for SMP included: presence of sinus septa, lower residual ridge height, thinner sinus membrane, thin mucosa biotype, previous surgical interventions, and smoking.
    • Thin sinus membrane, thin sinus membrane biotype, and reduced alveolar ridge height were significant risk factors for large SMPs (≥10 mm).

    Conclusions:

    • Sinus septa, thin sinus mucosa, staged procedures, and prior surgical interventions are key risk factors for SMP during LWT-based SFE.
    • A thin sinus membrane combined with a staged procedure significantly elevates the risk of large SMPs.
