Bone formation, or ossification, begins around the sixth to seventh week of embryonic development. Most bones develop from a cartilaginous template through the process of endochondral ossification. Summary

Dedifferentiated liposarcoma of the spermatic cord is rare, especially with ossification. Complete surgical resection with clear margins is the most effective treatment for this challenging diagnosis.

Background:

  • Liposarcoma is a malignant adipose tissue tumor.
  • Dedifferentiated liposarcoma is a rare subtype, typically affecting the extremities or retroperitoneum.
  • Spermatic cord liposarcoma is exceptionally rare, with no reported cases of macroscopic ossification.

Observation:

  • A patient presented with a painful, hard left suprapubic mass.
  • The mass was diagnosed as a spermatic cord tumor.
  • Histological examination revealed dedifferentiated spermatic cord liposarcoma with ossification and positive margins.

Findings:

  • The tumor required high orchiectomy for resection.
  • A second extended resection and adjuvant radiotherapy were necessary due to positive margins.
  • Preoperative differentiation of spermatic cord liposarcoma is diagnostically challenging.

Implications:

  • Complete surgical resection with clear microscopic margins is the most effective treatment.
  • Further research is needed to establish a gold standard treatment protocol.
  • This case highlights a rare presentation of dedifferentiated liposarcoma in the spermatic cord.
