Laparoscopic sleeve gastrectomy (LSG) significantly reduces thyroid hormones TSH and FT3, with FT4 levels remaining stable. Antral resection during LSG leads to greater weight loss and FT3 reduction compared to antral preservation.
Area of Science:
- Bariatric Surgery
- Endocrinology
- Metabolic Surgery
Background:
- Bariatric surgery's impact on thyroid hormones is inconsistent.
- Laparoscopic sleeve gastrectomy (LSG) is a common bariatric procedure.
- Thyroid hormone regulation is crucial in metabolic health.
Purpose of the Study:
- To evaluate thyroid hormone (TSH, FT3, FT4) changes after LSG.
- To compare LSG with antral resection (AR) versus antral preservation (AP).
- To assess the influence of weight loss on thyroid function post-LSG.
Main Methods:
- Prospective study of euthyroid obese patients undergoing LSG.
- Thyroid hormones measured preoperatively, at 1 month, and 1 year post-LSG.
- Comparison of outcomes between AR (n=58) and AP (n=48) groups.
Main Results:
- Significant decrease in FT3 and TSH levels post-LSG; FT4 levels unchanged.
- AR group showed significantly higher weight loss (BMI, %EBMIL, %TWL) than AP group.
- Greater reduction in FT3 levels observed in the AR group compared to the AP group.
Conclusions:
- LSG effectively reduces TSH and FT3, while FT4 remains stable.
- LSG with AR results in superior short-term weight loss and greater FT3 reduction.
- Antral resection may play a role in modulating thyroid hormone levels post-LSG.
