Background:

  • Immediate implant placement (IIP) is a common procedure in the aesthetic zone.
  • Long-term outcomes and risk factors for recession require further investigation.

Purpose of the Study:

  • To evaluate the 10-year clinical, aesthetic, and radiographic outcomes of single immediate implant placement.
  • To identify risk factors associated with advanced mid-facial recession after IIP.

Main Methods:

  • 22 patients received immediate implants in the aesthetic zone with flapless surgery and socket grafting.
  • Outcomes were assessed at 5 and 10 years, with CBCT scans at 10 years.
  • Connective tissue grafts (CTG) were used in 7 patients for specific deficiencies.

Main Results:

  • No significant differences in clinical parameters were observed between 5 and 10 years.
  • Mid-facial recession of ≥1 mm occurred in 33% of implants by 10 years.
  • Buccal shoulder position, no CTG, convex emergence profile, and central incisor position were putative risk factors for recession.

Conclusions:

  • Advanced mid-facial recession is a common long-term complication following immediate implant placement.
  • Careful consideration and patient selection are necessary for IIP in the aesthetic zone.
