Secondary Raynaud's phenomenon (RP) is linked to microvascular endothelial dysfunction. Early detection may identify patients at risk for connective tissue disease and cardiovascular disease (CVD).

- To investigate the association between Raynaud's phenomenon (RP) and microvascular peripheral endothelial dysfunction (PED) using a non-invasive NO-dependent method.

- Peripheral arterial tonometry was used to assess for PED (index <2).
- Raynaud's phenomenon (RP) presence was determined by chart review; secondary RP was analyzed.

- A significant association was found between secondary RP and microvascular PED (OR: 2.45; P=0.0236) after adjusting for confounders.
- This association remained significant in women.
- 95.1% of participants did not have RP (control group), while 4.3% had secondary RP.
Conclusions:
- Secondary RP is associated with microvascular PED detected by a non-invasive NO-dependent method.
- Early detection of microvascular PED may help identify individuals with secondary RP at risk for connective tissue disease and cardiovascular disease (CVD).
