Electrosurgical keratoplasty for keratoconus caused temporary flattening but significant stromal damage. Histopathology revealed destroyed Bowman's membrane and altered anterior corneal collagen.

Background:

  • Keratoconus management presents challenges for contact lens intolerance.
  • Electrosurgical keratoplasty was explored as an alternative surgical approach.

Observation:

  • A patient with keratoconus underwent electrosurgical keratoplasty due to contact lens intolerance.
  • Initial postoperative flattening of the central cornea recurred within five weeks.

Findings:

  • Pathologic examination post-penetrating keratoplasty showed destroyed Bowman's membrane.
  • Significant stromal scarring and collagen alteration were observed in the anterior two-thirds of the cornea.
  • The posterior cornea, Descemet's membrane, and endothelium remained unaffected.
