Pneumothorax is a rare complication of coronavirus disease 2019 (COVID-19). This case series highlights three patients with COVID-19 pneumonia who developed pneumothorax, suggesting inflammation-induced lung injury as a potential cause.

- The findings suggest that inflammation-induced pulmonary parenchymal injury and necrosis may lead to air leaks, causing pneumothorax in COVID-19 patients, similar to mechanisms observed during the 2003 SARS outbreak.
- Two patients were managed conservatively with chest tube drainage or observation, while one experienced fatal multi-organ system dysfunction.
