    Hereditary ovalocytosis, a red blood cell trait in Papua New Guinea, appears to offer resistance to severe malaria infections. This study investigated malaria incidence in individuals with ovalocytosis versus normocytes.

    Background:

    • Hereditary ovalocytosis is geographically linked to malaria-endemic regions.
    • This condition may provide a selective advantage against malaria.
    • Previous research suggests a potential protective effect of ovalocytosis against malaria.

    Purpose of the Study:

    • To investigate the incidence of malaria in Melanesians with known red blood cell morphology.
    • To determine the severity of malarial infections in individuals with and without hereditary ovalocytosis.
    • To explore the potential association between hereditary ovalocytosis and resistance to malaria.

    Main Methods:

    • Investigated malaria incidence in 1616 Melanesians with documented red blood cell morphology.
    • Assessed malaria infection severity in a subsample of the study population.
    • Compared parasitemia ratios between ovalocytic and normocytic individuals for different Plasmodium species.

    Main Results:

    • Ovalocytic individuals showed a tendency towards greater resistance to severe malarial infections compared to normocytic individuals.
    • Parasitemia ratios in ovalocytes versus normocytes were 1.05 for P. falciparum, 0.90 for P. vivax, 0.54 for P. malariae, and 0.91 for any species.
    • A reduced ratio of parasitemia was observed in ovalocytes, particularly for P. malariae.

    Conclusions:

    • Hereditary ovalocytosis may confer a degree of resistance to severe malaria in Papua New Guinea.
    • The study highlights challenges in definitively proving the selective advantage of ovalocytosis.
    • Further research is needed to fully elucidate the protective mechanisms and epidemiological impact of hereditary ovalocytosis against malaria.
