Summary

This case study highlights a rare instance of primary temporal bone diffuse B-cell non-Hodgkin lymphoma. Despite a significant lesion, the patient surprisingly had no cranial nerve deficits, leading to a lymphoma diagnosis after initial suspicion of infection.

Background:

Primary temporal bone non-Hodgkin lymphoma is an exceptionally rare condition.
Accurate diagnosis can be challenging due to varied presentations and potential mimics.

Observation:

A 71-year-old male presented with a month-long ulcerative mastoid lesion.
The lesion's significant size contrasted with the unexpected absence of cranial nerve deficits.
Initial clinical suspicion leaned towards infectious mastoiditis.

Findings:

Biopsies definitively diagnosed diffuse B-cell non-Hodgkin lymphoma of the temporal bone.
Differential diagnoses included squamous cell carcinoma, infectious mastoiditis, and actinomycosis.

Implications:

This case underscores the importance of histopathological examination for rare temporal bone malignancies.
Highlights the need to consider lymphoma in the differential diagnosis of aggressive mastoid lesions, even without neurological compromise.
Contributes to the limited literature on primary temporal bone lymphomas, aiding future diagnostic and treatment strategies.
