Area of Science:

  • Oncology
  • Radiotherapy
  • Medical Imaging

Background:

  • Adaptive radiotherapy (ART) involves replanning radiation treatments to address anatomical variations during therapy.
  • Current methods using standard imaging may not fully optimize treatment due to tumor mobility and volume changes.

Purpose of the Study:

  • To review the rationale, feasibility, and benefits of using PET and/or MRI to guide ART.
  • To explore the application of functional and soft-tissue imaging in optimizing ART across various cancer types.

Main Methods:

  • Review of current literature and clinical data on PET- and MRI-guided ART.
  • Focus on tumor localizations including lung, head and neck, cervical, and prostate cancers.

Main Results:

  • PET and MRI offer enhanced visualization of tumor mobility and volume variations, enabling more precise ART.
  • Initial data suggests PET- and MRI-guided ART is feasible and safe, with promising results in specific cancer types.
  • Extensive clinical experience is currently limited.

Conclusions:

  • Functional imaging (PET) and soft-tissue imaging (MRI) show potential for optimizing adaptive radiotherapy.
  • Further Phase I/II studies are essential to validate efficacy and safety.
  • Cost-effectiveness analyses and randomized trials are needed before integrating into routine practice.
