Summary

This study found no significant links between diffusion-weighted imaging (DWI) and muscle biopsy results in myositis patients. Larger sample sizes are needed to explore the potential of DWI in diagnosing muscle disorders.

Background:

  • Diffusion-weighted imaging (DWI) shows promise for assessing muscle disorders.
  • Histopathologic changes in muscle diseases can be reflected by DWI findings.

Purpose of the Study:

  • To investigate associations between muscle histopathology and DWI in patients with myositis and other myopathies.
  • To evaluate apparent diffusion coefficients (ADC) using histogram analysis in relation to biopsy findings.

Main Methods:

  • Retrospective analysis of 19 patients with myopathies.
  • Apparent diffusion coefficients (ADC) evaluated via histogram analysis of biopsied muscle.
  • Histopathology assessed using established scoring systems and immunohistochemical staining (MHC, CD68, CD8, CD4).

Main Results:

  • No statistically significant associations were found between ADC histogram parameters and histopathology scores.
  • Trends observed for skewness with Tateyama score (p=0.14) and kurtosis with MHC staining (p=0.07) did not reach significance.
  • ADC parameters did not correlate with CD68 or CD8 positive cells; a trend for skewness with CD4 cells (p=0.07) was noted.

Conclusions:

  • The study did not identify statistically significant associations between DWI and histopathology in muscle diseases.
  • A small patient sample size may have limited the ability to detect significant correlations.
  • Discrepancies may arise from DWI reflecting overall cellularity versus histopathology scores targeting specific disease aspects.
