Summary

External cephalic version (ECV) success is hindered by maternal obesity, breech descent, low amniotic fluid, and posterior fetal back position. Breech descent and posterior fetal back position independently impact ECV outcomes.

Area of Science:

  • Obstetrics and Gynecology
  • Perinatal Medicine
  • Fetal Medicine

Background:

  • External cephalic version (ECV) is a procedure to turn a fetus from a breech to a cephalic presentation before labor.
  • Identifying factors influencing ECV success is crucial for improving obstetric outcomes and reducing Cesarean births.

Purpose of the Study:

  • To prospectively identify factors associated with the success or failure of external cephalic version (ECV).
  • To analyze the independent impact of specific clinical variables on ECV outcomes.

Main Methods:

  • Prospective data collection on 67 patients undergoing ECV after 37 weeks' gestation.
  • Inclusion of fetal monitoring, ultrasound, and tocolysis within the ECV protocol.
  • Statistical analysis (chi-squared) to determine associations between patient/fetal factors and version success.

Related Experiment Videos

Main Results:

  • Overall ECV success rate was 60% (40 out of 67 patients).
  • Significant factors associated with ECV failure included maternal obesity, breech descent into the pelvis, decreased amniotic fluid, and posterior fetal back position.
  • Active labor did not affect success rates unless breech descent had occurred.

Conclusions:

  • Maternal obesity, breech descent, low amniotic fluid, and posterior fetal position are significant predictors of ECV failure.
  • Breech descent and posterior fetal back position demonstrate independent predictive value for ECV success, even after controlling for other variables.
