Summary

A minor acetabular fracture, often missed by X-rays, can cause severe hip pain due to blood buildup. Draining this blood (hemarthrosis) can relieve pain and improve hip motion.

Background:

  • Hip pain following trauma can be debilitating.
  • Acetabular fractures are fractures of the hip socket.
  • Radiography is the standard initial imaging modality for hip injuries.

Observation:

  • An 81-year-old woman presented with severe hip pain post-fall.
  • Initial radiography was normal.
  • Computed tomography (CT) and sonography revealed a minor, displaced acetabular fracture with hemarthrosis.

Findings:

  • Aspiration of 15 ml of blood relieved intracapsular pressure from 30 kPa to atmospheric.
  • The patient experienced significant pain relief and increased joint motion post-aspiration.
  • This case highlights that acetabular fractures can be radiographically occult.

  • Acetabular fractures may present insidiously, complicating diagnosis.
  • Hip joint tamponade from hemarthrosis is a potential consequence.
  • Aspiration of hemarthrosis may serve as a palliative and therapeutic intervention for hip joint tamponade.
