Background:

  • Renal artery stenosis (RAS) is a significant cause of kidney disease.
  • Accurate assessment of renal involvement in RAS is crucial for patient management.
  • Non-invasive imaging techniques are needed to evaluate kidney parenchyma changes.

Purpose of the Study:

  • To investigate kidney diffusion properties in patients with RAS using diffusion tensor imaging (DTI).
  • To assess the utility of DTI in detecting and quantifying renal parenchymal alterations associated with RAS.

Main Methods:

  • Prospective study involving 35 RAS patients and 15 controls.
  • Diffusion tensor imaging (DTI) was performed to acquire apparent diffusion coefficient (ADC) and fractional anisotropy (FA) values.
  • Comparison of cortical and medullary ADC and FA values between RAS groups (50-75% and >75% stenosis) and controls.

Main Results:

  • Reduced cortical ADC was observed in both RAS groups compared to controls.
  • Reduced medullary FA was significant only in the >75% RAS group.
  • No significant correlation was found between ADC/FA values and estimated glomerular filtration rate (eGFR).

Conclusions:

  • DTI can detect alterations in renal diffusion parameters caused by RAS.
  • DTI offers a non-invasive, repeatable method for evaluating renal parenchyma in RAS patients.
  • DTI findings could be valuable for the diagnostic evaluation of RAS.
