Background:

  • Bilateral renal cell carcinomas (RCCs) with distinct histological subtypes are exceptionally rare in medical literature.
  • Synchronous occurrence of different histological types of RCC in both kidneys presents a unique diagnostic and therapeutic challenge.

Observation:

  • A 62-year-old male patient presented with bilateral renal tumors exhibiting varied imaging characteristics on contrast-enhanced computed tomography and magnetic resonance imaging.
  • Surgical intervention involved partial resection of both renal tumors.

Findings:

  • Histopathological and immunohistochemical analyses confirmed clear cell renal cell carcinoma (ccRCC) in the left kidney.
  • The right renal tumor was diagnosed as mucinous tubular and spindle cell carcinoma (MTSCC).

Implications:

  • This case represents the first documented instance of simultaneous ccRCC and MTSCC in bilateral kidneys.
  • Understanding the synchronous presentation of diverse RCC subtypes is crucial for accurate diagnosis, treatment planning, and prognosis in renal oncology.
