Area of Science:

  • Neurology
  • Obstetrics
  • Critical Care Medicine

Background:

  • Severe pre-eclampsia poses significant risks during pregnancy, including neurological complications.
  • Intracerebral hemorrhage and venous sinus thrombosis are rare but life-threatening obstetric emergencies.

Observation:

  • A 24-year-old primigravid woman at 29 weeks gestation presented with symptoms of pre-eclampsia, rapidly progressing to intracerebral hemorrhage and superior sagittal sinus thrombosis.
  • Neurological deterioration necessitated urgent surgical intervention.

Findings:

  • The patient underwent the first reported simultaneous decompressive hemicraniectomy and cesarean section.
  • This combined approach was crucial for managing the life-threatening maternal neurological condition and ensuring fetal well-being.

Implications:

  • Simultaneous decompressive hemicraniectomy and cesarean section represent a novel, life-saving intervention in complex obstetric emergencies.
  • This case highlights the critical need for multidisciplinary collaboration in managing severe maternal morbidity.
