    Background:

    • Dental implants are crucial for full-mouth reconstruction.
    • Understanding implant healing is vital for long-term success.
    • Novel implant designs require rigorous histologic evaluation.

    Purpose of the Study:

    • To histologically assess human bone healing around a unique triangular-neck dental implant.
    • To quantify bone-to-implant contact (BIC) in a submerged healing model.
    • To evaluate the efficacy of a narrower implant neck design compared to the implant body.

    Main Methods:

    • Four patients received full-arch prostheses supported by multiple implants.
    • Eight customized, reduced-diameter triangular-neck implants (Ø 3.5 × 8 mm) were placed in healed edentulous ridges.
    • Implants were retrieved after 6 months of submerged healing for microcomputed tomography (μCT) and undecalcified histologic examination.

    Main Results:

    • All retrieved implant gaps were filled with mature, remodeled bone.
    • Mean BIC in buccolingual/buccopalatal and mesiodistal sections were 64.45% and 65.39%, respectively.
    • Mean 3D BIC was 68.58%, with no significant difference between μCT and histologic measurements.

    Conclusions:

    • The triangular-neck dental implant demonstrated successful osseointegration in human subjects.
    • The unique design facilitates mature bone infill within the implant neck gap.
    • Histologic findings confirm the efficacy and positive healing response of this novel implant design.
