Background:

  • The USMLE Step 1 scoring changed to pass/fail in February 2020 to reduce emphasis on performance and improve student well-being.
  • This study investigates the impact of this change on radiology residency selection.

Purpose of the Study:

  • To determine the perspectives of Diagnostic Radiology (DR), Interventional Radiology (IR), and Nuclear Medicine (NM) program directors (PDs) on the USMLE Step 1 pass/fail scoring system.

Main Methods:

  • A survey was distributed to 179 DR, 84 IR, and 34 NM PDs from ACGME-accredited programs.
  • The survey included Likert scale, demographic, and free-text questions.

Main Results:

  • 140 responses were analyzed (47.1% response rate).
  • A majority (69.6%) of PDs disagreed with the change.
  • Most PDs (90.7%) believe pass/fail Step 1 makes applicant comparison harder, with increased emphasis on Step 2 scores (89.3%) and medical school reputation (72.7%).

Conclusions:

  • The long-term effects of pass/fail Step 1 scoring remain uncertain.
  • Many radiology PDs do not support the change, questioning its ability to achieve the intended goals of reduced emphasis and improved well-being.
