Esophageal Varices-II: Clinical Features and Management01:28

Esophageal Varices-II: Clinical Features and Management

265
Esophageal varices often manifest as gastrointestinal bleeding episodes, presenting symptoms like hematemesis (vomiting of blood), hematochezia (passing fresh blood via the rectum), and melena (black, tarry stools). Other signs can include weight loss, anorexia, abdominal discomfort, jaundice, pruritus, altered mental status, and muscle cramps.
In the initial assessment, a thorough review of the patient's medical history is vital to identify risk factors such as liver disease, alcohol...

Diseases of the Liver and Gallbladder

1.7K
Liver and gallbladder diseases are a significant health concern, with prominent conditions including cirrhosis, hepatitis, non-alcoholic fatty liver disease (NAFLD), and gallstones. Jaundice is a common manifestation of liver and biliary disease.
Cirrhosis is characterized by the scarring of hepatic lobules in the liver, which are replaced by fibrous tissue, affecting the liver's normal functioning. NAFLD, on the other hand, is caused by an excessive build-up of fat in the liver, not...

Effect of Hepatic Disease on Pharmacokinetics: Pathophysiologic Assessment and Liver Function Test

101
In clinical practice, the direct measurement of hepatic blood flow to evaluate liver function presents significant challenges due to the intricate and specialized nature of the necessary techniques. Consequently, healthcare professionals often rely on empirical estimates derived from thorough patient examinations and liver function tests to gauge liver health. Among the tools at their disposal, the Child–Pugh and MELD scoring systems stand out for their ability to categorize and assess...

Summary

Acute liver failure (ALF) can signal advanced pancreatic cancer. This case highlights hepatic decompensation from an occult pancreatic malignancy, emphasizing the need for thorough investigation in unexplained ALF.

Area of Science:

  • Hepatology
  • Gastroenterology
  • Oncology

Background:

  • Acute liver failure (ALF) is infrequently caused by metastatic liver disease.
  • Early detection of metastatic disease is crucial for patient outcomes.

Observation:

  • A 64-year-old male presented with symptoms suggestive of advanced disease, including abdominal distension, appetite loss, and significant weight loss.
  • Admission labs revealed severe acute kidney injury, hyperbilirubinemia, elevated liver enzymes (AST, ALT), and markedly elevated alkaline phosphatase.
  • Abdominal MRI demonstrated liver metastases, pancreatic cystic lesions, and ascites.

Findings:

  • Ascitic fluid analysis confirmed adenocarcinoma.
  • Elevated cancer antigen 19-9 (CA 19-9) levels supported the diagnosis.
  • The patient experienced rapid deterioration, succumbing to cardiac arrest.

Implications:

  • This case underscores that acute liver failure can be a rare presenting sign of occult pancreatic malignancy.
  • Highlights the importance of considering advanced gastrointestinal cancers in the differential diagnosis of unexplained acute liver failure.
  • Emphasizes the aggressive nature and poor prognosis associated with metastatic pancreatic adenocarcinoma presenting as ALF.
