Pseudomonas aeruginosa is the most frequent cause of chronic suppurative otitis media (CSOM), a persistent ear condition. This study identified it in 58% of cases, highlighting its significance in ear infections.
Area of Science:
- Otolaryngology
- Microbiology
- Infectious Diseases
Background:
- Chronic suppurative otitis media (CSOM) is an ear disease characterized by a perforated tympanic membrane lasting over 6-12 weeks.
- Identifying the causative pathogens is crucial for effective treatment and management of CSOM.
Purpose of the Study:
- To determine the primary bacterial causes of chronic suppurative otitis media (CSOM) at our institution.
- To provide etiological data that can inform clinical practice and antimicrobial stewardship.
Main Methods:
- Ear swabs were collected from 139 patients diagnosed with CSOM.
- Microbiological culture and sensitivity testing were performed on all samples.
- Statistical analysis was conducted using SPSS version 26.
Main Results:
- The most common pathogen identified was Pseudomonas aeruginosa, present in 81 (58%) of the cases.
- Klebsiella pneumoniae was the second most frequent pathogen, found in 58 (42%) of the cases.
- These findings indicate a predominance of Gram-negative bacilli in CSOM at this center.
Conclusions:
- Pseudomonas aeruginosa is the predominant causative agent of CSOM in the study population.
- The prevalence of Klebsiella pneumoniae also warrants attention in the management of CSOM.
- Understanding local microbial patterns is essential for targeted therapy of chronic ear infections.

Etiology
Three primary contributing factors have been identified.
Chronic Pharyngitis
Etiology
It often arises from persistent viral or bacterial infections affecting sinuses and tonsils.
Additional contributing factors include inadequate dental hygiene, mouth breathing, recurring tonsillitis, allergic rhinitis, laryngopharyngeal reflux, and exposure to smoke, chemicals, and other environmental pollutants. Allergic reactions to pollen, mold, and pet dander, chronic cough, excessive voice usage,...
