Pyogenic vertebral body osteomyelitis is rare in children. This case highlights successful conservative management with antibiotics for a 10-year-old boy, showing complete recovery without long-term effects.

- The patient experienced a complete recovery with no symptoms or activity limitations at 12-month follow-up.
Implications:
- PVBO should be considered in pediatric patients with atypical spinal or hip symptoms.
- Magnetic Resonance Imaging (MRI) is crucial for diagnosis, and prompt antibiotic therapy ensures favorable outcomes without sequelae.
