Background:

Gallbladder sarcomatoid carcinoma is a rare malignancy with limited understanding of its clinical course.
Characterizing this aggressive tumor is crucial for improving patient outcomes.

Observation:

A retrospective analysis of seven cases (1997-2017) revealed a median age of 68.5 years, with 85.7% female patients.
Most tumors exceeded 7 cm, and advanced TNM staging correlated with poor prognosis and rapid post-surgical progression.
One stage IIIB patient receiving gemcitabine/capecitabine adjuvant chemoradiotherapy achieved 12-month PFS and 15-month OS.
