Depression and suicidal thoughts in untreated and treated narcolepsy: Systematic analysis

Background:

• Narcolepsy type 1 (NT1) is a chronic neurological disorder affecting sleep-wake regulation.
• Depressive symptoms and suicidal thoughts are frequently observed in NT1 patients, but their determinants and treatment impact require further investigation.

Purpose of the Study:

• To determine the prevalence and risk factors of depressive symptoms and suicidal thoughts in adults with NT1.
• To evaluate the changes in these symptoms following NT1 management.
• To identify risk factors for major depressive episode (MDE) and suicide risk (SR) in NT1.

Main Methods:

• A cohort of 297 NT1 patients and 346 controls were assessed using the Beck Depression Inventory-II (BDI-II).
• A subset of 101 drug-free NT1 patients completed the BDI-II during treatment.
• The Mini International Neuropsychiatric Interview was used to diagnose MDE and SR in 162 NT1 patients.

Main Results:

• NT1 patients exhibited higher BDI-II scores than controls, with untreated patients scoring higher than treated patients.
• Moderate to severe depressive symptoms were linked to lower education, obesity, severe narcolepsy symptoms, autonomic dysfunction, and poorer quality of life.
• Suicidal thoughts were more prevalent in untreated NT1 patients (22.7%) compared to controls (12.4%) and were associated with male gender and symptom severity. MDE and SR were diagnosed in 18.1% and 16.9% of patients, respectively.

Conclusions:

• Depression, depressive symptoms, and suicidal thoughts are prevalent in NT1, especially in untreated individuals, and correlate with disease severity.
• Effective management of NT1 leads to significant improvements in depressive symptoms and a reduction in suicidal thoughts.
