Postoperative Nursing Management for Kidney Transplant PatientsPostoperative nursing management care includes monitoring the surgical site, encouraging early movement, and promoting lung health through breathing exercises. Nurses also administer prescribed medications like H2-blockers, such as famotidine, or proton pump inhibitors, like omeprazole, to help prevent gastrointestinal ulcers and bleeding. Fungal infections in the mouth and bladder can result from immunosuppressive and antibiotic...

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Summary

Litigation in nephrectomy cases is rising. While preoperative negligence is most common, perioperative errors, particularly vascular injuries, lead to the highest settlement payouts in urologic surgery.

Area of Science:

  • Urology
  • Medical-Legal Studies
  • Surgical Litigation

Background:

  • The medical-legal landscape presents increasing concerns for surgical specialties, including urology.
  • Understanding litigation factors in oncologic nephrectomy is crucial for risk management and patient safety.

Purpose of the Study:

  • To systematically review and summarize factors contributing to oncologic nephrectomy litigation.
  • To identify elements influencing verdicts and settlements in these cases.

Main Methods:

  • Systematic review of publicly available verdict reports from Westlaw (1990-2019).
  • Search term: "nephrectomy".
  • Analysis of defendant specialty, alleged negligence, complications, outcomes, and indemnity payments by two independent reviewers.

Main Results:

  • 103 cases analyzed; 78% involved radical nephrectomies.
  • Preoperative negligence was the most frequent claim (48%), but perioperative negligence yielded the highest average payment ($5,493,151).
  • Perioperative claims (41% of cases) were often linked to vascular injury (46%); this association was statistically significant (p=0.042).

Conclusions:

  • Perioperative negligence, despite being less frequent than preoperative claims, incurs the highest financial settlements in nephrectomy litigation.
  • Insights highlight critical management stages in renal cell cancer surgery susceptible to legal action.
