Clozapine augmentation with Electroconvulsive therapy (ECT) shows promise for treatment-resistant schizophrenia. A majority of patients improved and remained out of hospital for a year, supporting further research into this combined therapy.
Area of Science:
- Psychiatry
- Neuroscience
- Pharmacology
Background:
- Schizophrenia treatment resistance poses a significant clinical challenge.
- Clozapine is effective but can be insufficient for some patients.
- Augmentation strategies are crucial for refractory cases.
Purpose of the Study:
- To evaluate the efficacy of clozapine combined with Electroconvulsive therapy (ECT) in patients with schizophrenia resistant to clozapine treatment.
- To assess treatment response and rehospitalization rates following clozapine-ECT (C+ECT) combination therapy.
Main Methods:
- Retrospective analysis of electronic health records for patients receiving C+ECT.
- Identification of patient cohorts treated with C+ECT over a 10-year period.
- Assessment of treatment response using Clinical Global Impression-Improvement (CGI-I) scores and 1-year rehospitalization rates.
Main Results:
- 76% of patients (n=32) demonstrated at least minimal improvement (CGI-I ≤3) after C+ECT.
- 64% of patients experienced no adverse events during treatment.
- 75% of responders and 70% of non-responders remained out of hospital at 1-year follow-up.
Conclusions:
- Clozapine augmentation with ECT (C+ECT) demonstrates effectiveness in clozapine-resistant schizophrenia.
- Findings align with existing meta-analyses and randomized controlled trials.
- Further randomized controlled trials are necessary to solidify recommendations for C+ECT.
