Background:

• A large, pedunculated malignant mesenchymoma of the bladder, exhibiting fibroleiomyomatous, myxomatous, osteoid, and cartilaginous differentiation, was surgically resected.
• Recurrence was noted six months post-surgery, followed by multimodal treatment including surgery, chemotherapy, and irradiation.
• Autopsy confirmed extensive local recurrence, widespread metastases, and an incidental finding of clear cell renal cell carcinoma.

Findings:

• Malignant mesenchymoma of the bladder can present aggressively with rapid recurrence and metastasis.
• The tumor demonstrated a complex histological composition, including bone and cartilage formation.
• Coexistence of bladder malignant mesenchymoma and renal clear cell carcinoma was observed, a novel association.
