Summary

Physical activity levels, specifically moderate-to-vigorous physical activity (MVPA), are linked to Modic changes (MC) in men, particularly MC2. However, this association was not found in women, suggesting sex-specific effects of MVPA on lumbar spine health.

Background:

  • Modic changes (MC) in the lumbar spine are potential causes of low back pain (LBP).
  • Risk factors for MC include male gender, smoking, high BMI, strenuous work, and physical activity (PA), though PA's role is debated.
  • Previous studies on PA and MC have yielded contradictory findings, necessitating further investigation.

Purpose of the Study:

  • To investigate the association between device-measured moderate-to-vigorous physical activity (MVPA) and lumbar Modic changes (MC).
  • To clarify the role of MVPA in the development of lumbar spine abnormalities.

Main Methods:

  • 1374 participants from the Northern Finland Birth Cohort 1966 were analyzed.
  • Moderate-to-vigorous physical activity (MVPA) was measured using accelerometers at ages 46-48.
  • Lumbar magnetic resonance imaging (MRI) was used to identify MC, with associations analyzed using sex-stratified logistic regression.

Main Results:

  • In men, increased MVPA showed a positive association with any MC and specifically with MC2.
  • For every 60 minutes/day of MVPA, adjusted odds ratios were 1.41 for any MC and 1.54 for MC2 in men.
  • In women, a positive association between MVPA and MC1 was observed only before adjustments.

Conclusions:

  • Increased MVPA is associated with higher odds of any MC and MC2 in men.
  • No independent association was found between MVPA and MC in women.
  • Findings suggest a sex-specific relationship between physical activity and Modic changes in the lumbar spine.
