    Background:

    • Blunt abdominal aortic injury (BAAI) is a rare but life-threatening condition.
    • Understanding the epidemiology and clinical presentation of BAAI is essential for timely diagnosis and management.

    Observation:

    • This review analyzes 33 cases of blunt trauma to the abdominal aorta.
    • Motor vehicle accidents account for approximately 50% of reported cases.
    • Associated injuries are common, present in 71% of patients.

    Findings:

    • Diagnosis is often made in the immediate or early period (69% of cases).
    • Injuries most frequently involve the inferior mesenteric artery (33%) and renal arteries (24%).
    • The overall mortality rate for blunt abdominal aortic injury is 27%.

    Implications:

    • Lower extremity ischemia on physical examination is a key diagnostic clue.
    • Aortography is valuable for diagnosing BAAI when suspected but without distal ischemia.
    • Nonoperative management should be reserved for minimal intimal disruptions only.
