Area of Science:

  • Radiology
  • Rheumatology
  • Medical Imaging Analysis

Background:

  • Radiographic joint space narrowing (JSN) assessment in rheumatoid arthritis (RA) is crucial but often relies on complex, time-consuming visual methods.
  • Developing automated tools can enhance the efficiency and accuracy of monitoring RA progression.

Purpose of the Study:

  • To validate a novel automatic software system designed to evaluate the temporal changes in joint space narrowing (JSN) in rheumatoid arthritis (RA) patients.
  • To compare the software's JSN evaluation with traditional methods like the modified Total Sharp Score (mTSS).

Main Methods:

  • A cohort of 39 RA patients treated with tocilizumab underwent hand radiography at 0, 6, and 12 months.
  • An automatic system was developed to detect joint locations and calculate the joint space difference index (JSDI), representing chronological JSN change.
  • JSN was assessed by both the automatic system (JSDI) and a musculoskeletal radiologist using mTSS.

Main Results:

  • The automatic software's JSDI indicated significantly greater JSN progression between 0 and 12 months compared to 0 and 6 months (p < 0.01).
  • No significant changes in mTSS scores were observed across the 0, 6, and 12-month intervals.
  • Patients with higher baseline disease activity showed significantly higher JSDI between 0 and 6 months (p = 0.02).

Conclusions:

  • The developed automatic software is capable of evaluating JSN progression in the finger joints of RA patients from X-ray images.
  • This automated approach offers a potentially more efficient and objective method for monitoring disease progression in RA compared to manual assessments.
