An Esophagogastroduodenoscopy (EGD) is a diagnostic procedure in which an endoscopist uses a flexible, lighted endoscope to visualize the upper gastrointestinal (GI) tract. The procedure includes visualizing the oropharynx, esophagus, stomach, and the first part of the small intestine, the duodenum.
During an EGD, the endoscope can be used to:

Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Area of Science:

  • Gastroenterology
  • Medical Devices
  • Diagnostic Procedures

Background:

  • High-resolution esophageal manometry (HRM) is the gold standard for diagnosing esophageal motility disorders.
  • Catheter placement via the nares can be difficult due to coiling and gastroesophageal junction (GEJ) passage issues.
  • Endoscopy-guided placement improves success but can still be hindered by anatomical variations.

Observation:

  • The fragility and cost of HRM catheters preclude using standard biopsy tools for advancement.
  • No existing literature describes using endoscopic accessories to aid difficult HRM catheter placement.
  • A unique case involved using a Roth Net through the endoscope's suction channel.

Findings:

  • The Roth Net successfully facilitated the advancement of the esophageal manometry catheter into the stomach.
  • This technique was employed during direct endoscopic visualization to overcome placement challenges.
  • The Roth Net provided a viable method for advancing the delicate catheter.

Implications:

  • This case report introduces a novel technique for improving HRM catheter placement success rates.
  • The Roth Net may serve as a valuable accessory for endoscopists facing difficult HRM placements.
  • Further research could explore the broader applicability of this method in clinical practice.
